Unless you choose otherwise, if you are enrolled in Health Care FSA as of your Separation Date, the Company will continue to deduct your regular Health Care FSA contribution from your severance pay on a pre-tax basis until the earlier of your Benefit End Date or December 31st of the year of your Separation Date. If your severance payments continue from one calendar year to the next, during Annual Enrollment you may elect Health Care FSA for the next Plan Year and amounts will continue to be taken from your severance pay until your Benefit End Date. After your Benefit End Date, you may elect to continue your Health Care FSA coverage on a post-tax basis under COBRA through the end of the calendar year in which your Benefit End Date falls or the last day during which you have Continuation Coverage, whichever comes first. The Benefit Service Center will send you COBRA information directly. If you have any questions about Continuation Coverage or you do not receive your COBRA information within one month after your Separation Date, please contact the Benefit Service Center at 1-800-565-0122. You may submit claims for expenses incurred during any period that you continued to contribute to your Health Care FSA. Smart-Choice. The HSA is a tax-favored savings account that you own, and it helps you save for health care expenses with pretax money you set aside. You control how you spend the funds, just like any purchase decision you make. Unused dollars in your HSA roll over from year to year. There is no “use it or lose it” rule. Fresh Thyme will contribute funds to your account if you elect to participate. You must log into the portal and elect the HSA amount. If you only want the employer portion, please enroll in the HSA but elect a $0.00 contribution. Fresh Thyme will make a $250 contribution for an individual account, and a $500 contribution for family coverage. You will be eligible to contribute to an HSA if you satisfy all of these IRS conditions: • You must be enrolled in the HSA $3,000 plan. • You must not have additional coverage under another health plan unless it is a qualified HDHP (review your spouse’s plan, other employer, TRICARE, or IHS plans carefully). • You must not be claimed as a dependent on another person’s tax return (spouses are typically not dependents; however, there are some cases where this applies, such as disability). • You must not be entitled to benefits under Part A or Part B of Medicare. IMPORTANT NOTE If you are already enrolled in an HSA, any unused funds will rollover in your account. However, you must make new contribution elections for 2023 if you so desire and still meet the eligibility requirements.
