Journal of Hand Surgery (British and European Volume), 26(3), 261-263.This prospective, randomized, single-blind study compared the use of a hand-based thermoplastic metacarpal brace with use of neighbor strapping of the fourth and fifth digits for treatment of nondisplaced, minimally angulated fifth metacarpal neck fractures. Outcome measures included pain evaluation using a verbal rating scale, active and passive flexion and extension at the fifth MCP joint, and total ROM of the fifth digit. Participants also rated their overall satisfaction during the 3 weeks of immobilization. Participants in the brace group complained of significantly less pain during immobilization and had slightly better digit mobility following healing as compared with the strapping group; the brace group returned to work earlier and reported higher levels of satisfaction. A hand-based custom orthosis is an acceptable treatment option for minimally displaced fifth metacarpal neck fractures. Postoperative treatment of metacarpal fractures—Classical physical therapy compared with a home exercise program. Journal of Hand Therapy, 31(1), 20-28.The authors designed a prospective, cohort, randomized, controlled trial to evaluate whether a home exercise program or traditional physical therapy was more effective in the postoperative management of metacarpal fractures. Their study included 60 patients suffering from digital metacarpal fractures. Fractures of the first metacarpal were not included. All patients were prospectively randomized into either the physical therapy group or the home exercise group. Follow-up evaluations occurred at 2, 6, and 12 weeks postoperatively. After 2 weeks, the ROM in both groups was still severely reduced. Twelve weeks after surgery, the total digital flexion ROM of the involved digit improved to 245 degrees (physical therapy group) and 256 degrees (home exercise group). We had another busy week, but tried to squeeze in a few minutes of extra recess here and there to soak up this nice weather we've been having. Here's what the kids have been working on in class:Reading:Strategies to improve their fluency: - pay attention to punctuation marks - notice special types of print (bold words, capitals, italics) - read dialogue and know who is talking - read dialogue the way the author wants you to (said, shouted, cried, asked) - stop to think during reading, and write your thinking on a sticky note! - asking questions before, during and after reading.At home, help your child remember these strategies when they do their reading homework.Writing:The kids are drafting their personal narrative. We have worked on how writers: - follow a plan when they write - describe the setting using the 5 senses - stretch out important actions step - by - stepMath:Last week, we finished Unit 1 and took the end of the unit assessment. I will share your child’s score at upcoming conferences. So far in Unit 2 we have worked on: - counting 3 digit numbers by grouping into ones, tens and hundreds - learning what place value is (ones, tens, hundreds) - determining totals using place value - adding 3 digit numbers using place valueScience:In science, we are continuing to learn about maps. Last week, we focused on what a scale is and how a scale is used on a map. In the pictures, you will see the kids playing a vocabulary game with this unit’s vocabulary. These are the vocabulary words they need to know: map, landform, bodies of water, key/legend, symbol, compass rose and scale.
